I have been chasing this problem for three weeks. One whole reel of Prusa filament has disappeared with no completed print!  The answer for me was the Bowden connector at the top of the heatbreak.  A quick strip down to remove and reposition the PTFE tube, this time under tension so the tube could not "float" and life is back to normal.  Thanks Everyone for the Posts which led me to the solution.
