What Went Wrong With This Print?

I've been working on my first real print.  Really simple but something I actually have a use for.  Earlier versions have printed fine.  But today this happened (photos below).  Note in the photos that the part on the left is one which is going very wrong; the one on the right is an earlier print of an earlier version of the part.  I stopped the printer when I saw that there was a problem.  The first photo is of the underside of the parts.  

Note too that the skirt is different than the skirts used in all earlier version of this part.  And this different skirt was caused by the slicer (PrusaSlicer). 

Re the skirt I do not have a clue.  But I wonder if the print got messed up because the spool of filament was not revolving easily enough?  It's mounted exactly the same as it has been for earlier prints.  But when I noticed the problem the filament looked pretty taut.

Is that object actually and totally flat on the print bed?

Not really, as long as it flows smoothly into the machine.

Is the part laying on the build plate flat? From your slicer picture looks like the part isn't sitting on the build plate everywhere. Can you give us a picture of the sliced part from the back?

That part was obviously not flat on the bed. You can tell from the skirt being printed only around part of the first layer. You need to use the place to bed tool to make sure the part in flat. Also get into the habit of looking at the preview of the slice, that way you will see immediately if there are any potential issues like this.   It’s very noticeable on the picture of the preview you provided. It’s actually a useful reason to print with a skirt even if you don’t need it to prime the nozzle.

Also the first version the middle section is all bridged from what I can tell.
