This an under-extrusion issue?

I am not sure how fix this print issue.  I rebuilt the hotend/extruder assembly and everything appears great in there.  I have a few other mk3s's and no issues with those.  Ideas what is going on here and how to fix it?

Hard to say what adjustment will fix it. If the same gcode and filament is printing well on one Mk3s it really should do about as well on another Mk3S.

But it does look like underextrusion. And that can be many things. If the Bondtech isn't aligned exactly centered on the filament diameter, less idler tension and it is skipping, Bondtech gears has debris in them, a tight fitting PTFE or debris in the heat break, a slightly clogged nozzle from printing something higher temp that hasn't been fully removed, ...

The fact you rebuilt the hot end - hints you probably missed a step on reassembly. Did you follow the E3D-V6 assembly guide; or did you follow the Prusa guide? Did you apply thermal compound to the heat break? Did you do the high temp torque?

Sorry I should have been more clear, I rebuilt it due to the print issues, same results after rebuilding it.  I am using the same gcode and filament in all my prusa printers with no issues other than this particular printer.

I did apply thermal paste to the heatbreak, I have only ever printed pla, nothing else.  I followed the online prusa guide.

Read the E3D-V6 guide - look for anything you didn't do. Even the stuff that seems trivial - because it isn't.

ps: Several Mk3S+ printer are suffering unexplained underextrusion right now. Prusa is just telling users they need to jack up the extrusion multiplier; and honestly, that appears to be the only resolution at this point. Whether it is motor drive currents, some minor deviation in hotends, fan speeds, thermistor curves, who knows. But it's frustrating.
