Stubborn underextrusion

I've started having a under-extrusion issue. To trouble shoot I have cleaned the nozzle with the acupuncture needle and preformed four cold pulls until I saw a cone at the end of the filament pulls. I have tried optimizing the live-z offset (-1.080 to -1.090) and increased the flow rate to 100-115%. Adjusting z or the flow rate solves the problem temporarily but usually shows up again on the opposite side of the print bed.

Need more info - nozzle temp, material type, brand (prusament requires higher temps).

Nozzle temp was the standard 215 material is prusament PLA. I think I've got it working now. The roll of filament has been sitting around for a while so I raised the temperature to 240 and lowered the z offset. Working well now.
