Poor layer filling

I convinced a friend to buy a MK3S+ and he happily sent my a video of him playing with one of his first prints.

He was happy but he did not realize how poor his layers were filling up and I have not had this issue in my 14 days of print time on mine.

He printed from the gcode file he found on the Prusa SD card.

Is the only way to fix this by increasing the Flow rate on the tuning menu while it is printing (I can't image his internal layers)?

There are also start/stop gaps in his external perimeters.

If the first layer is correct, it affects the result in future layers. And you should not try to compensate for the problem with other settings in Prusa slicer. Then your problems will probably get even worse.
There are two things that cause the most common problems, and that is an incorrect Z-level and a poorly cleaned bed.
There are lots of posts about this, and the answers are always the same.

The picture shows that your Z level is far too high.
Recalibrate the first layer so that you get a smooth surface without any gaps.

Z level does effect the first layer but from that pic the first layer is set to high.  Without having a good foundation to print on the rest of the layers will be effected.  There's nothing to change in the gcode but you need to get this dialed in on your printer BEFORE printing any of the included gcode files, or any gcode file.
