Anyone know what may have happened here?

Hey everyone,

Super weird, I've been having so many failed prints lately. I just performed a full maintenance on the machine - minus recalibration because i've heard you don't need to recalibrate if you have a good first layer. 

This latest failed print was even more weird than the last ones because everything was going fine until about the 5th layer when it just started extruding beside the print... anyone know what may have gone wrong here?

I might as well be the first.
Check your X-axis grubscrews , align one of the screws with the axles flat part and tighten that one, then tighten the other one

That does look like layer shifting. Adding to the good advice from @Ringarn67, belt tension and linear bearing lubrication are other things to check. Try disabling the steppers (X & Y should move freely) and move the X & Y axis slowly by hand and feel for any friction or uneven movement. Pluck the belts and see if they vibrate a bit. There are some guides on properly tightening belts, but generally, if they "strum" like a guitar string, tension is good. Do check to see if you're getting crash detection errors.

They were pretty tight and aligned but I un-tightened them and then retightened them just in case
