Under-extrusion issues

So I'm having some fairly dramatic under-extrusion issues and I have no idea what the issue is. I'm using s3d as a slicer. It seems like the infill is good, but for some reason the outer shell is a trainwreck.

After repeatedly messing with the slicer settings (increasing outer shells, increasing extrusion rate, etc...), ran the file through Netfabb, and even used meshmixer to "make solid". I inspected the mesh closely and it looks fine, yet I was still having problems, so I printed a calibration cube which came out fine.

So I changed the filament to the original Prusa filament and tried again. The situation improved (ignore the ringing, I need to replace my bearings and tighten the belts as I have done 0 maintenance on this printer in the past year), but there are still some blatant under-extrusion problems on the surface.

The infill looks good, so it looks like its just a surface issue. It was even to the point where I suspected that I had the wrong nozzle size/type for the outer shell or something weird like that, but no everything looks fine. I even uninstalled my slicer and reinstalled an updated version, same issue.

I realize I should post my settings (can't do it now but can upload later). but I was wondering if anyone had any similar problems with surface issues that might point me in the right direction to WTF.

Welp, in case anyone is wondering, I decided to risk it and print a different huge project anyways and it is printing fine, so it looks like the problem is with the model or the slicer. Hopefully this is of use to someone someday.
