Printing trouble on multiple parts

I am using PLA and I can't figure out why this happens. This also occurs when I print the included Prusa print. It starts off okay and then this happens. Any suggestions would be much appreciated.

Loose allen screw on the x motor, or not tightened on the flat of the motor shaft first and then the second screw.

@billcampbell

Thanks for the reply and I will definitely give this a try tonight. Are you suggesting the x motor is too tight? I notice the bed does make a noise when it moves along the x. Just curious how this affects the print and/or filament extrusion.

I was suggesting the pulley attached to the x motor is slipping. The bed slides on the y axis but your layer shift is along x.

If the pulley is not loose on the shaft of the motor, then the belt may be slack but the amount of movement is consistent with drive pulley rotation.

It can also be bearings that are not lubricated and seizing up on the rods. Extruder should move easily across the X axis rods with no sticking or tight spots.
