Underextrusion problem. Basic Troubleshooting didn't help.

I run 7 Prusa MK2s and MK3s printers at my school.  My older MK2s is underextruding filament.  It previously had a heatbed error which I repaired.  Now it is underextruding.  

I replaced the nozzle,  increased the tension of the springs, decreased the tension on the springs, checked the fan.  Nothing has improved the issues.  Other than completely rebuilding the extruder, is there anything else I should look at?

(There have been no other changes, same Slic3er, firmware is updated, other printers work fine with standard print settings.)

Factory reset and check the extruder gears are clean and properly attached to the motor.
